Strategies of preconception care of women with Existing Diabetes

In the diabetic pregnancy complicated by significant hyperglycemia, the fetal environment is disturbed by an excess of glucose transported from the maternal compartment to the fetal circulation. The adverse metabolic conditions involve more than glucose imbalance. There are also alterations in amino acids levels, ketone production and fatty acid metabolism that can affect the pregnancy. However hyperglycemia may be more poorly tolerated by the fetus and with increasing evidence that poor control is responsible for still birth and congenital malformations seen in pregnancies complicated by diabetes, it is apparent that preconception care reduces the incidence of fetal complications. Strategies for widespread implementation of preconception care assumed that patient would be recruited to attend centralized specialist diabetes clinics because it was noted that tight control of diabetes is often quiet difficult to achieve.
