THE TALE OF IMMUNOGENETIC PATHOGENESIS OF DIABETES
BY
HALA EL-SHEREEF
LECTURER OF INTERNAL MEDICINE-ASSIUT UNIVERSITY , Egypt
q Type 1diabetes
4 phases can be distinguished
Genetic predisposition
Environmental factors
Autoimmune response
Beta cell destruction
Genetic
factors: several genes may be involved
* MHC complex :3 classes of genes in this complex
Type 1 is associated with HLADR3 and HLADR4
HLADR2 protects against type 1.
* Several other loci with weaker linkage
Environmental factors:
* Viruses
* Diet
* Life style
Autoimmune response : leading to production of auto antibodies and
beta cell destruction .
q
Type 2 diabetes
Is predominantly or perhaps entirely due to genetic factors
several genes are implicated
q
MODY
Definite gene defects have been identified, all are inherited in an
autosomal dominant manner
q
Genetic susceptibility to diabetic complications
Siblings of probands with diabetic nephropathy show much higher
incidence of nephropathy compared with probands without
nephropathy
q
Genetic counselling
